This form is used to cancel a Pass/Fail request and revert back to a graded status. The course will then be recorded with a grade on your grade report and transcript.

Deadlines and Submissions

 An official request needs to be submitted to Academic Services, York Hall C102 or scanned and emailed to acadservices@glendon.yorku.ca, before the deadline to drop a course

Note: It is the student’s responsibility to notify the course director of this change.
